Boosting Your Children's Vision

Vision is the sense that arrives last and develops most after birth, so a newborn who's staring straight through you is doing exactly what she should.

It's also the area where one finding outweighs everything else here, and where I live it isn't an abstract one. We're raising five children in Asia, where short-sightedness in children has risen faster than almost anywhere in the world.

What develops when

A newborn sees best at about twenty to thirty centimeters, roughly the distance from the crook of your arm to your face. That isn't a coincidence.

She prefers high contrast to color, and faces to everything else. Over the following months focus sharpens, the eyes learn to work together, color vision fills in, and depth perception arrives at around five to eight months, which is also when reaching becomes accurate.

Occasional crossing or drifting of the eyes is normal in the first three or four months while the muscles coordinate. After about four months, mention it. An eye that turns consistently at any age needs looking at, and never waiting out.

The outdoor finding

If you read one section here, this is it.

Time spent outdoors during childhood consistently goes with a lower chance of developing myopia, and the effect isn't small. A randomized trial in Guangzhou added forty minutes of outdoor time to the school day and reduced the number of children who became short-sighted over the following three years, and other trials have pointed the same way.

The mechanism is light intensity. Outdoor light is many times brighter than indoor lighting even on an overcast day, and bright light influences how the eyeball grows. It doesn't seem to be about looking at distant things, which was the old explanation. It's about being outside. I'm told the researchers were as surprised by that as anybody.

The working number most researchers land on is around two hours a day. In a hot climate that means early morning and late afternoon, and shade counts. A child under a tree is still getting far more light than a child indoors. This is the best thing here and it costs nothing.

Screens

I have a fourteen-year-old, a twelve-year-old and a six-year-old, so I'm not going to pretend our household has solved this. What the evidence supports is fairly narrow: screens haven't been shown to damage eyes directly, what they do is displace outdoor time, and near work of any kind for long uninterrupted stretches speeds myopia progression, so the problem is real and the mechanism is mostly indirect. The practical version: no screens under eighteen months beyond video calls, very limited and shared use to about two, and after that the thing to protect is the outdoor hours. Take a break every twenty minutes to look at something far away for twenty seconds. Keep screens out of the bedroom. Be realistic. A rule you can't keep is worse than a looser one you can.

How the rest develops

What helps at each stage

Most of it is ordinary interaction, which is reassuring, and none of it requires anything you have to buy.

  • Newborn: hold her at twenty to thirty centimeters and let her look at your face. High-contrast black and white patterns are more interesting to her than pastel ones. Change which side you hold her on so both eyes work.
  • Two to four months: slowly move an object across her field of view and let her track it. Talk while you do it so she has sound and sight together.
  • Four to eight months: floor time, reaching, and things to grasp at different distances. Depth perception develops through reaching, so give her something to reach for.
  • Eight months to two years: rolling a ball, stacking, posting shapes into holes. All of it is hand-eye coordination and all of it is play.
  • Two years and up: outdoors, daily, as much as the climate allows. Then books, drawing and puzzles, with breaks.
  • At every age: protect the eyes from direct sun with a brimmed hat, and sunglasses for older children outdoors for long periods.

Signs that need an eye test

Vision problems in small children are easy to miss, because a child with poor sight in one eye has no idea anything is unusual. She has never seen anything else.

  • An eye that turns in or out consistently, at any age, or any turning at all after about four months.
  • Eyes that don't follow a face or an object by around three months.
  • White or unusual reflection in the pupil in a photograph, which needs prompt assessment.
  • Persistent watering, crusting, redness, or unusual sensitivity to light.
  • Head tilting, one eye closed or covered, or sitting very close to look at things.
  • Frequent eye rubbing outside of tiredness, headaches, or clumsiness that seems out of step with her age.
  • A family history of squint, lazy eye or strong prescriptions, which deserves raising even with no symptoms.

Have a routine check by around three to four, before school, since that's the age most treatable problems are found and treated. Amblyopia responds well to treatment in early childhood and much less well later, so this is one of the few areas where being early changes the outcome.
